During my father’s end-of-life care, decisions were made to de-escalate his treatment—including limiting interventions such as antibiotics—without clearly informing him or his substitute decision-makers, my brother and me. We were never told that his care had been reduced, nor were we asked to provide informed consent. Instead, we were told that he had only “days to weeks” to live, without being given a clear explanation of why.

This was especially troubling because my father had previously expressed his wish to continue life-sustaining treatment and pursue transfer to a chronic ventilator facility. Our concern is not simply that a medical decision was made, but that decisions affecting his life and death were made without transparency, meaningful consultation, or respect for his documented wishes.
