My dad, George Pruner, was a kind and loving man who spent over a year in the hospital, including months in the ICU. Despite his critical condition, he had a chance at continued life through a ventilator care facility - a difficult journey but one we discussed in our goal meetings with healthcare providers. Yet, doctors began making decisions about his care without my family's knowledge, ultimately choosing to de-escalate treatment. Our dad passed away without ever agreeing to this situation. 

We later discovered discrepancies in the hospital's chart notes - what was said to us did not align with what was being recorded. If our family had access to these notes in real time, we could have identified these inconsistencies and advocated for our dad's care before it was too late. 

We are calling for a simple but impactful change: healthcare providers must upload real-time chart notes to a secure app that families can access. This transparency would ensure families are fully informed and involved in critical medical decisions, preventing situations like the one we have endured.
