Our investigation of your claim reveals the following facts: Your first office visit with Dr. Mark Swancutt occurred on November 1, 2010. At that visit, Dr. Swancutt wore his identification badge which clearly indicated that Dr. Swancutt is employed by UT Southwestern Medical Center at Dallas (UTSW). During that visit, Dr. Swancutt received a set of records from you which he took with him to UTSW and locked in a cabinet in his office.
Apparently, the patient is now responsible for examining the doctor's ID badge before consenting for treatment? And why would a doctor leave the premises with a patient's health records of that facility just because he is an employee of another facility where he will NEVER see that patient?
