

Petition Update – In Memory of Cheryl Raw

Every signature, every comment, and every share means more than words can say.
My mum, Cheryl Raw, died on 12 January 2025, just nine days after being discharged from hospital while suffering from liver failure and severe mental distress.
She should never have been sent home — and sadly, she’s not the only one.
Since launching this petition, I’ve spoken with other families who’ve faced the same heartbreak: loved ones discharged too soon, without the proper checks, assessments, or support in place. These are not isolated incidents — they are signs of a system that is failing its most vulnerable patients.
My campaign calls for a Mandatory Integrated Discharge Protocol for all patients with both mental and physical health needs.
This would mean:
Full multi-disciplinary assessment before discharge
Mandatory family consultation and written safety plan
Post-discharge checks and safeguarding for all high-risk cases
I’m preparing to present this evidence, along with my mum’s story, to local MPs and NHS representatives ahead of a formal investigatory meeting.
My goal is to ensure her death is not ignored, and that meaningful change follows.
