As you may know, my mum, Cheryl Raw, passed away earlier this year. She had both physical and mental health needs, but she was discharged from hospital too soon, without proper assessment or support. Our family wasn’t consulted — and within weeks, she was gone.

I’ve launched a petition to call for systemic change in how vulnerable patients with both mental and physical health needs are discharged from hospital. No one else should have to suffer the way my mum did, or the way we as a family still do.

This petition calls for:

Proper risk and capacity assessments before discharge
Family/advocate consultation when patients are vulnerable
Clear, safe discharge plans including medication and follow-up care
Safeguarding oversight for at-risk patients
