- 10:10 a.m.: Faith was transported to the nurse’s office for a scheduled feeding, remaining in her stroller. Staff confirmed her feeding tube was intact after the session.
- 11:45 a.m.: Staff member Megan Shaffer transported Faith to another cubicle for a diaper change.
- Minutes later: Shaffer reportedly shouted, “Oh my gosh, the tube is out, the whole tube is out.”
This timeline raises serious questions:
- What happened during the 95-minute gap between the feeding and the diaper change?
- How could the feeding tube be "intact" at 10:10 a.m. and then "the whole tube is out" minutes after 11:45 a.m.?
