Another unnecessary death and another family grieving.
Another morass of questions that will be ignored, blocked, diverted and manipulated.
Today I’ve been busy writing a letter to the Care Quality Commission’s Dr Kevin Cleary.
I was a mum to an amazing young man. Matthew Leahy, who was aged just 20 when he entered an EPUT NHS Mental Health Unit - for his own safety and to begin recovery. He was dead 7 days later.
Since his horrendous, needless death, I have been studying.
EPUT NHS and its predecessors NEPFT and SEPT have an illuminating history.
Dating way back to 2004, I have proven that ligature points have not been removed, despite recommendations, leading to the additional, avoidable deaths of 11 patients under the care of North Essex Partnership University Trust.
A GUILTY plea has been submitted by the Trust under Section 3 Health and Safety.
To be heard at Chelmsford High Court 16th June 2021.

Nevertheless, patients are still ligaturing and dying on known ligature points which SHOULD and have NOT been removed almost 20 years later!
Why is this being ignored!
Ongoing multiple deaths by ligature clearly show the Essex Partnership is still failing its patients.
How many GUILTY pleas before any change?
