Sehr geehrter Herr Bundesrat Alain Berset

Die HIV-Prä-Expositionsprophylaxe (PrEP) ist ein wichtiger Bestandteil, um die WHO-Ziele zur Beendigung der HIV-Epidemie bis zum Jahre 2030 zu erreichen. Im Jahre 2019 startete das SwissPrEPared-Programm mit dem Ziel, die Auswirkungen der PrEP in der Schweiz zu untersuchen. Seitdem konnten wir auch in der Schweiz einen historischen Rückgang der HIV-Diagnosen beobachten, wie das bereits in anderen Teilen der Welt nach Einführung der PrEP der Fall war.

Das Potential der PrEP wird aktuell noch nicht voll ausgeschöpft. Das SwissPrEPared-Programm hat nicht nur gezeigt, dass die PrEP unter medizinischer Begleitung effektiv ist, sondern auch, dass sie sicher ist. Es wurde aber auch sichtbar, dass in der Schweiz vor allem Menschen mit gutem Einkommen und hohem Bildungsniveau die PrEP nutzen. Für einen Grossteil der Risiko-Population sind die Hürden zu hoch. 

Eine dieser Hürden sind die Kosten: Anders als in vielen anderen Ländern dieser Welt, darunter fast die gesamte EU, muss die PrEP in der Schweiz von den Anwender*innen selbst bezahlt werden. Auch für die medizinischen Kontrollen gibt es keine eindeutige Regelung.

Das müsste nicht sein: Bereits im Juni 2020 wurde von der Hersteller-Firma Gilead der Antrag gestellt, die PrEP als Pflichtleistung in den Leistungskatalog der obligatorischen Krankenkasse aufzunehmen. 

Dieser Antrag wurde bis heute nicht von Ihnen bearbeitet! 

Wir fordern Sie daher auf zu veranlassen, dass dieser Antrag noch in diesem Jahr zu geprüft wird. Jede weitere Verzögerung bedeutet neue HIV-Infektionen, die mit der PrEP vermeidbar wären.

La PrEP (prophylaxie pré-exposition) est un outil essentiel pour atteindre les objectifs de l'OMS de mettre fin à l'épidémie de VIH d’ici à 2030. L'étude SwissPrEPared a débuté en 2019 dans le but d'étudier l’impact de la PrEP en Suisse. Depuis lors, il y a eu une baisse historique du nombre de personnes ayant découvert vivre avec le VIH en Suisse, cela a déjà été le cas dans d'autres parties du monde après l'introduction de la PrEP.

Le potentiel de la PrEP n'est pas pleinement exploité actuellement. Le programme SwissPrEPared a montré que la PrEP avec un accompagnement médical est efficace mais également sûre. Cependant, l'étude montre également qu'en Suisse, ce sont surtout des personnes disposant de bons revenus et d'un niveau d'éducation élevé qui prennent la PrEP. Pour une grande partie de la population et notamment les personnes les plus exposées au VIH, les obstacles sont encore trop élevés.

Contrairement à de nombreux autres pays, dont la quasi-totalité de l'UE, en Suisse les bénéficiaires doivent encore prendre à leur charge le médicament pour la PrEP.  Il n'y a pas non plus de règlementation claire concernant le suivi médical.

Cela ne devrait plus être le cas : dès juin 2020, le fabricant Gilead a déposé une demande pour que la PrEP soit incluse dans la liste des prestations relevant de l'assurance obligatoire des soins (dite « assurance maladie de base »). A ce jour, cette demande n’a toujours pas été traitée par vos services !

Nous vous demandons de faire en sorte que cette demande soit examinée avant la fin de cette année. Tout retard supplémentaire sera responsable de nouvelles infections à VIH qui auraient pu être évitées avec la PrEP.

The HIV Pre-Exposition-Prophylaxis (PrEP) is an integral element to reach the WHO goal to eliminate HIV until the year 2030. In 2019, the program SwissPrEPared started with the aim to examine the impact of the PrEP in Switzerland. Ever since, we could observe an historical decline of HIV infections in Switzerland, a similar development that also happened in other regions of the world after the introduction of PrEP.

Yet the potential of PrEP has not been fully utilized. Not only did the SwissPrEPared program show that PrEP under medical guidance is effective, it is also safe. However, it also became evident that in Switzerland, mostly people with stable income and a high education use PrEP. For the majority of the HIV high-risk groups the hurdles are still too high.

One of these hurdles are the costs: Other than in other parts of the world, including almost all countries in the EU, in Switzerland the PrEP has to be paid by the users themselves. Furthermore, there are no consistent guidelines for the medical consultations either. 

This does not have to stay this way: In June 2020 the biopharmaceutical company Gilead has made an official proposition to include PrEP as a mandatory benefit of the compulsory health insurance in Switzerland.

Until this day this proposition has not been answered!

We therefore demand for you to set the inquiry of this proposition in motion by the end of this year. Any further delay means more unnecessary HIV infections that could have been prevented with PrEP.
