We received information on 8/3/21 that beginning 8/29/21, Service Facilitation will have to be conducted in-person, at your home.  We want to be transparent in saying your Service Facilitator will see several families in a day, going from home to home.

As providers, we are concerned for your health, especially with the emergence of the Delta variant of COVID and your inherent risk. We want to advocate for being able to continue our visits using Telehealth (video methods), as we have been for the past 16 months.
