Dear Sir/Madam,

I am submitting a formal complaint regarding the veterinary care of my dog, Elva Richards, who tragically passed away on 21 July 2025 following emergency pyometra surgery at Vets4Pets Leeds Birstall.

Among the clinical records I received, there is a handwritten annotation reading “tampon up bum”. This entry is uninitialled, undated, and unexplained, with no clinical justification provided.

The absence of context raises serious concerns as to whether this act was performed while Elva was alive, and if so, whether it may have caused unnecessary suffering or constituted an unlawful procedure under the Animal Welfare Act 2006.

I respectfully request that the RSPCA review this matter as a potential breach of animal welfare legislation and take appropriate action.
The annotation ‘tampon up bum’ appears in Elva’s records. It is uninitialled, undated, and unexplained. If this act was performed while she was alive, it may constitute potential breaches of the Animal Welfare Act 2006, including unnecessary suffering (Section 4), unlawful mutilation (Section 5), and a failure in duty of care (Section 9).”
Kind regards, Mrs Sarah Richards
