Zhang Zhan has been on indefinte partial hunger strike since her trial in December 2020. Before that she was on a full hunger strike for six months to protest her arrest. Her health continues to deteriorate. In July 2023, her weight dropped to 37kg in July 2023. She has been admitted to prison hospital with severe malnutrition, gastrointestinal disease, and low white blood cells. 

2023年8月30日，李大伟律师与张展的母亲通话后得知，张展已又一次因身体虚弱住进了监狱医院，白血球低，肠胃病严重；她在7月送院前体重降至37公斤，只有被捕前的一半。

张展自2020年5月在武汉被抓后绝食抗议，被强制灌食；当年12月受审并当庭被判刑四年后开始半绝食，直到现在。2021年8月即两年前她也曾因严重营养不良送院并被强制灌食，当年10月一度命悬一线，引起国际社会强烈关注。中国当局的继续关押，导致拒绝屈服认罪的张展健康状况持续恶化。

9月2日是张展的生日，这个生日她将在监狱医院度过。
