When the criminal justice system fails, who investigates the whole system?

No statutory body examines failures across the whole criminal legal process spanning police, prosecutors and courts. This includes witness contamination or Section 28 of the Youth Justice and Criminal Evidence Act 1999 breaches.

We want an independent body to establish facts, identify failings and refer misconduct to relevant authorities. Read more here:
