The Issue

Hello America,

We need to stop big problems at the "All of Us" research program, paid for with $1.5 billion of American tax dollars. This petition was started because of how Azucena Fuentes ruined the program. Patients are getting hurt, and the staff is creating fake research data that will mess up the study's results.

--> Problems:

Patient Bleeding and Injury: There have been many incidents where patients experienced painful, unprofessional blood draws, leading to bleeding and trauma. On December 18, 2023, a patient reported a "dangerously incompetent" blood draw that was "long, painful and bloody."

 

 

On December 18, 2023, the patient even sent pictures of their bruised and bloody arm but Azucena made no changes to stop patient injury.

 

 

Fake Data: In 2022 verified fake patient measurements were entered into the research database, no changes were made to supervise enrollments and All of Us Research data since then may still be at risk of being fake under Azucena Fuentes management. 

  • Program Manager Azucena Fuentes refused to assign the three Program Coordinators to the clinics for supervision of blood draws or staff training. This caused patient injuries, while also leaving the research staff so unsupported they resorted to creating fake data.

 

 

Toxic Workplace: Employees who speak up against clinic mismanagement have faced retaliation and termination. 

Government Waste: Staff members leave early and stay home with full pay 

  • Wasted Money: The 3 "Program Coordinators" responsible to manage clinic staff instead stay home, do not attend team meetings and refuse to be in the clinics training staff despite injuries & fake data reports
  • Staff Leaving Early:  Azucena Fuentes' secretary leaves work more than an hour early every day. This is a big problem because it's stealing public grant money that was meant to pay for a full day’s work. When other staff see this happening they leave early too.
  • Clinics Without Staff: Because unsupervised staff leave early, clinics don't have coordinators working. This is really bad for older people who travel a long way to get help, and nobody is there.

Nothing Has Changed: A Patient even directly emailed the National Institutes of Health about these issues on February 17, 2024, but there has been no change. Azucena has not been held accountable for causing injuries or damaging the program with fake data.

 

 

--> Change Needed:

Real Investigation: A public investigation into the mismanagement of the program under Azucena Fuentes that leads to corrective termination.


Patient Safety & Real Data: Immediate changes to stop patient injury and stop fake data recording. Stop including data collected under Azucena's management until its accuracy can be fully verified.


Transparency with Taxpayer Money: Provide clear and public communication about the investigation and corrective actions taken.

  • Despite serious issues like patient injuries, fake data, and multiple patient complaints under her management in just 2.5 years on the job, Azucena Fuentes presents herself as a "Senior Research Program Manager" on her LinkedIn profile.
  • On LinkedIn, Azucena promotes herself without mention of the patient injuries or fake data her management has caused. This large difference between her LinkedIn persona and the actual situation raises deep concerns about her grasp of the reality and if she has any remorse for the injuries or fake data.

 

 

  • June 2021 - Azucena Fuentes was made Program manager without others interviewed. The previous program manager left suddenly for a pharmaceutical company & the decision was made with no due diligence of Azucena's qualifications. The program quickly deteriorated with patient complaints becoming frequent and escalating in severity.
  • Azucena's previous experience is limited to administrative assistant at a DentAlign office and 1 year of weight loss research.
  •  After 2 years of Azucena's management the staff became unable to measure a patient's waist size or take blood samples without injury and complaints.
