Email them your social security card (back and front) and driver's license to CustomerInfoCenter@deo.myflorida.com , IdentityRequest@deo.myflorida.com , and AppealsCOMTRAC@deo.myflorida.com . This has worked for me .. i emailed them almost every day for 17 days til they emailed me back that I am identified now.
