Subject: Formal Complaint – PIP Process and Safeguarding Breach

Dear DWP Complaints Team,

I am writing to raise a formal complaint regarding the handling of my PIP claim, which I believe has breached my legal rights under the Equality Act 2010 and failed to meet safeguarding duties.

Specifically:
- Reasonable adjustments were not provided for my neurodivergent condition.
- There was no trauma-informed screening or safeguarding protocol applied during assessment.
- I was exposed to harm/distress without appropriate duty-of-care response.

These actions amount to discrimination under s.20/s.21 EA2010, and failure to uphold your Public Sector Equality Duty (s.149). I request a full investigation and immediate corrective action, including:
- A written apology and acknowledgment of breach.
- Reassessment under adjusted and safeguarded conditions.
- Disclosure of all assessment records under my right of access (GDPR Subject Access Request).

Please treat this as a formal complaint and respond within 14 days. Failure to resolve may result in escalation to ICE (Independent Case Examiner), the Parliamentary Ombudsman, or legal action.

Thanks to an advocate within Linkedin I am intending to complain against the DWP who refused to give me any reasonable adjustments in relation to my mandatory reconsideration request such as extra time and to send information via email, they also didn't give me any welfare or safeguarding protection despite knowing about previous attempts to end my life via Suicide.

I have now recieved my paperwork they made many assumptions and said I seemed well and engaged well despite being in bed, unwashed and in my Nighty, I also became emotionally unregulated, I can't help but feel they wanted me to reach MH crisis, or that I would decide i wouldn't pursue a mandatory reconsideration.
