Maastricht University Antipsychotic Withdrawal Survey, please help.

Have you taken antipsychotic medication (such as Zyprexa/Olanzapine, Seroquel/Quetiapine, Abilify/Aripiprazole, Risperdal/Risperidone, Haldol/Haloperidol, Stelazine/Trifluoperazine) for any condition or diagnosis, with or without other medications? Have you stopped or ever tried to stop? And are you currently 18 years or older?

The purpose of this research is to improve mental health services by better understanding antipsychotic medication withdrawal. Lead researcher is Will Hall, a therapist, PhD student, and former patient who has himself taken antipsychotics. The study is sponsored by Maastricht University School for Mental Health and Neuroscience in the Netherlands; co-sponsors include the International Institute for Psychiatric Drug Withdrawal.
