
Probation have acknowledged that they need to learn from this case and that they were failings.
The perpetrator was under 2 different probation services. This was due to a different borough handing Over to the borough he had been placed in. The lack of communication between the two is astounding. He was telling one probation officer he was struggling with his addictions and mental health whilst telling the other one he was fine and things were good. THEY DID NOT COMMUNICATE ONCE WITH EACH OTHER.
My aim is to now reach out to organisations that can help me put my plan into place as I feel being proactive is the only way this is going to push things out there. I really do believe in every HMO there should be a bleed kit, residents need to be safe so adequate individual risk assessments, and the biggest thing that has come from this is services need to be in contact with each other and share information
