Hundreds of people at a vulnerable time in their lives will benefit from the Hospital to Home program in the 2027 financial year. 

We’re pleased to share that we have received a commitment to continue the Hospital to Home model in WA for the coming financial year — an important step toward sustaining support for people transitioning from hospital back into the community. We appreciate the government’s support in making this possible.
