We have received a letter from UVA Health's Corporate Compliance & Privacy Office regarding a privacy breach involving Faith Johnson's protected health information following her passing. The letter confirms that on July 23, 2025, a grievance response document containing Faith's private medical details—including her name, address, medical history, diagnoses, and a description of the treatment she received leading to her death—was inadvertently emailed to another patient's parent. This accidental disclosure occurred as part of a grievance case process.

UVA Health has expressed its regret for this unfortunate incident and issued an apology. They have informed us that the recipient, another patient's parent (who is not a UVA Health physician), has confirmed deleting the erroneous email. UVA Health assures us that they have reinforced the importance of protecting patients' rights and privacy with the individual responsible for the mistake and are taking prompt action to prevent future occurrences.
