Since I was born in 2006, at least seven separate coroners’ Prevention of Future Deaths reports have been filed against Swansea Bay’s mental health services. Many are from just the past few years:

These reports paint a clear picture of chronic systemic failure — and little to no action taken. How many more people must die before this becomes a national scandal?
These reports document repeated, serious failures:
• Refusal of admission
• Neglect in care
• Breaches of duty
• Failure to follow risk management protocols
I was nearly the eighth death.

• I’m calling for a full, independent investigation into Swansea Bay Mental Health Services.
• I’ve submitted multiple formal complaints and a second draft report is due tomorrow (Wednesday 25th June)
• A forensic investigator is submitting key amendments — we must ensure the final report reflects the full extent of systemic failure and my lived experience.
• Without access to DBT, I remain at serious risk. This treatment isn’t optional — it’s urgent and life-saving.
