I’ve now submitted a detailed formal complaint to Health Care Inspectorate Wales (HIW) outlining systemic failures at Ward F (Swansea Bay Health Board) that severely endangered my life and continue to pose a risk to other vulnerable patients.

My letter highlights:

  • Severe safety breaches, including a suicide attempt via Just Eat delivery containing a deadly allergen, and another incident where I absconded through an unsecured office window despite being detained and actively suicidal.
  • Emotional and psychological harm caused by staff, including gaslighting, denial of PRN medication in clear distress, false statements made to both myself and my family regarding my care and plans going forward, and retaliatory harmful comments by nursing staff.
  • Mismanagement, lack of communication, failure to act on care needs, and a general culture of defensiveness and cover-up that allowed serious risk to life to go unchecked.
  • The deeply troubling conduct of Dr. Robertson, who made harmful, stigmatising and dismissive remarks that caused lasting emotional harm. She repeatedly minimised my experiences, showed no compassion, and fostered a climate where my concerns were ignored and my credibility undermined. Her behaviour had a chilling effect on my ability to speak openly about my distress — directly impacting my safety and recovery.
  • Lack of support for myself and still lack of clear plans for my care & treatment going forward
  • A retaliatory and baseless capacity assessment done the day after Wales Online reported my story. The doctor conducting it literally said:
    “Don’t know why to be honest... I was just told to do it.”
     It’s clear this was ordered from above — an abuse of clinical tools to silence a whistle blower. My solicitor and advocate both confirmed there was no lawful basis for the assessment. I was rightly found to have capacity at the end of the assessment but it never should've happened.

Tomorrow (Friday 6th June), I’ll be meeting Clare Taylor, Head of Forensic Mental Health Services, to discuss the draft response to my formal complaint. I’ll be making it absolutely clear that I expect:

  • Full transparency — no sanitised or vague wording.
  • Accountability for named staff where serious failings occurred.
  • A clear explanation of what systemic changes will be made so that what happened to me doesn’t happen to anyone else.
  • Clear treatment recommendations to be followed for me going forward

If the report lacks honesty, I will escalate directly to the Public Services Ombudsman for Wales. HIW, my solicitor, complaints & mental health advocate, and Members of the Senedd Rebecca Evans as well as Tom Giffard are all being kept well informed.

The risks at Ward F are not historical — they are ongoing. There have already been three coroners’ reports involving this ward. How many more lives have to be lost before there is real accountability?

Therefore, I am calling on Health Care Inspectorate Wales to conduct an urgent, independent investigation into the practices, culture, and leadership at Ward F and Swansea Bay Health Board’s mental health services. This investigation must be thorough and transparent, with clear outcomes that safeguard vulnerable patients.

I nearly died because of these failings. I won’t be silenced. Not now, not again.

I also want to be honest: speaking out has come at a cost. I’ve been subjected to targeted abuse on social media for calling out these issues. That only proves how urgently we need change. I will not stop speaking up despite this.

Meanwhile, my recovery is still being delayed. I’ve now been told that I need another assessment for DBT — because the one supposedly done on Ward F wasn’t completed properly. After all the trauma, I’m once again being made to wait due to their incompetence. I still need support, and I need people to keep standing with me while I fight for it.

My education and life continues to be on hold until I get the support that I urgently need. I call upon Swansea Bay Health board to urgently review plans for my care and give me a clear way forward so I can get my life back on track. Many thanks to Olchfa School for their continued support and a letter to support my deferral for university. 

I also again want to recognise the difference in care since I have come to Cefn Coed Hospital and thank the staff here for their support, although they have no influence over community mental health treatment matters. Neither less, It should be noted that since moving to Cefn Coed Hospital I have been far less emotionally heightened and suicidal due to staff here supporting me, acknowledging my needs and acting to meet them. They provide a high standard of support including in times of distress. There is a clear contrast - ward F staff antagonised me, neglected my needs, and put me further at risk whilst Cefn Coed staff are fully supportive and compassionate towards myself which has reduced my risks.

To amplify the campaign further, I will soon be using WriteToThem to call upon supporters to directly contact Senedd representatives, demanding action and accountability for mental health services in the Swansea Bay region.
