Launch a Full Public Inquiry into the death in custody of Anthony ‘Terry’ Roberts

The Issue

  • On 21st April 2018. Anthony ‘Terry’ Roberts was remanded in custody at Highdown
  • Prison, Surrey. He died the following day after a catalogue of failures on the part of prison staff to safeguard his health. Mr Roberts suffered from COPD and asthma and was left alone locked in his cell with breathing difficulties after a COPD attack. Prison staff then caused a series of delays in getting him to hospital and hampered the delivery of life saving procedures once at hospital which led to his untimely demise.

On the 23rd December 2020, Surrey Coroner’s Court concluded the inquest into his death with a narrative verdict citing ‘missed opportunities’ only. We Terry’s family were shocked and disappointed that the Coroner did not direct the court to investigate unlawful death or pursue an inquiry into their conduct of those whose care Terry was in. 

The court had heard contradictory evidence from the prison staff and that they had withheld important evidence from the court. Several witness statements were not completed and never investigated. Surrey police were called to Epsom general hospital following Terry’s death but these officers did not take any statements so could therefore not provide these in court.

Why would Surrey police not take statements when called to A&E dept after my loved one died?
Why was No CCTV by Epsom general a&e depart & highdown prison Surrey not handed over when we requested this several times?  the coroner said ‘ we don’t need this’ whys that? let me & my legal team be the judge of that.
Why did the external investigator not request cctv footage from our investigation yet all other investigations before & after my loved one was killed 22/4/2018, cctv was produced for other families though only a couple of minutes cctv recordings are shown leading up to their loved ones death? See the very long list unreported deaths in custody prisons Uk & Wales in updates & how often these are occurring on a monthly basis absolutely disgusting.
Why was the body worn cameras that were implemented in 2017 for prison employees not handed over either? 
Why was my loved one held in a prison carpark on a blue light ambulance & they the prison staff wouldn’t open the gate but we’re allowing other vehicles to leave & enter what sort of carry on is this? 
Why was my non violent loved one, low risk-risk assessed double cuffed & chained, unable to stand, speak or walk, had an oxygen mask on & was wheeled to a waiting blue light ambulance unable to leave and a radio response Highdown prison confirmed they didn’t receive? Why was our complaint of homicide to Surrey police not investigated properly?
Why did the prison nurse say in court we placed him in the cell on the bed but yet when x2 paramedics arrived they both said he was in distress trying to breath leaning up against a wall outside the cell? Male paramedic I asked my colleague to go get the ambulance chair straight away. Why was evidence re: this altercation with this Epsom General A&E nurse deliberately withheld by all within the resus bay at Epsom general hospital Surrey as well as x2 paramedics that were also in the resus bay not forgetting the female prison officer, & the other incompetent prison officer standing outside the resus bay with a mobile phone in his pocket? These two prison officers deliberately withheld this information from their prison statement as well & their recorded interviews.

The Last person to be cross examined via video link was this female prison officer who was laughing in court with two of her colleagues & who was in court almost every day leading up to the last week of our 4 week inquest. She was sitting in the public gallery with other witnesses listening to all the evidence of her colleagues but yet when it was her turn to be cross examined she had tonsillitis hence why the coroner agreed she be interviewed via video link why? She should’ve been ordered back into court. After hearing new evidence,  police should’ve been called into court & that didn’t happen either. She was chained to my loved One & refused to remove the cuffs and chains when he died & went into full cardiac arrest which is why she was assaulted by a qualified nurse at the A&E dept Epsom general hospital Surrey, pushed up against the resus bay equipment punched in the arm & called a heartless bitch. Why would this qualified A&E nurse feel the need to take such strong action against this female prison officer if she didn’t do anything wrong. But She did! She refused to remove the cuffs and chains when My Loved One Died Wearing them & died alone with no family present!  This was the first time we & everyone had heard this evidence in court as this was deliberately withheld from both her statements to Highdown prison & also her recorded interview. She hadn’t tonsillitis at all she was frightened to appear in court incase I done the same to her as she did to my love one but I wouldn’t or she was frightened she would be arrested for her inhumane treatment her lies & cover up. Why was our 4 week inquest not stopped immediately & properly investigated when hearing this new evidence? Why was the police not called in when other witnesses evidence was not matching the evidence from the paramedics when they arrived at Highdown prison?
All these witnesses should’ve been called back in to be re cross-examined as well as the prison governor & another custodial manager that never were interviewed that didn’t happen either.
Why did this female prison governor come to our family home with a family liaison officer when this prison governor was involved in this incident and who authorised the use of cuffs and chains on a critically I’ll man. Why did she after having her meetings with the two prison officers when they returned to Highdown prison from Epsom general Hosp Surrey come to our family home? This should never have happened. Why did she not speak to me at our front door & when she seen the camera doorbell she moved out of view. When the family liaison officer spoke to me at my front door she asked could we come in I refused. I knew something was wrong as they don’t do home visits. I stepped outside my front door with my house keys in my hand. I closed the door. She introduced herself and introduced the prison governor who I didn’t know at the time but was involved by authorising the use of the cuffs and chains. I said no bad vibes aloud whatever you need to say you say it here your not getting in. She said ok then and started to say at 11:15am this morning Terry ran into problems with his breathing he was seen by the prison nurse and had a nebuliser. Between 2 & 2:15pm Terry started to struggle with his breathing again and an ambulance was called. He was taken to hospital & had a cardiac arrest in the back of the ambulance on the way to hospital, at this point I thought she was going to say he’s in intensive care adults & Epsom general hospital the visiting times are etc but she didn’t. She said between 4:15 & 4:30pm Terry sadly passed away. I started to continuously Pat my chest and look out at a distance. She asked have you any family in London, we need to get you inside. She then started asking me questions what did I work at? Is anyone inside? What did Terry work at? Is there anyone I can call. I gave her my mobile phone and code to get in. I named 3 people in my contacts for her to call. Two people from church and another person. I told her to walk up the street to speak to them. She apparently said to all three that Terry had sadly passed & I went into shock when she told me. All three people arrived at our family home one person travelling from kent, one from Clapham & one from North London. Terry death was recorded at 4:56pm. The prison governor & family liaison officer arrived at 7:46pm. 
Our daughter was inside ready for bed as she had school in the morning. When I went into the room she said mammy you are breathing heavy, I said don’t worry. I said mammy has two people she knows downstairs. I introduced my child to these two individuals and said I worked with them. She was playing with her toys in the sitting room. People started to arrive.
The family liaison officer & prison governor waited in the kitchen. When our family friend a minister at church arrived from Kent he asked who they were. My daughter went back upstairs to bed. I was asked from my friend did you tell my daughter I said no & I didn’t want too. He said you need to tell her sooner rather than later. This was the hardest thing I had to do in life to tell our daughter that daddy had gone to heaven. She came back down stairs when I spoke to her and hugged her asking was she ok she nodded. She walked into the sitting room she sat & played the piano as she & her father use to play the piano together all the time. She had only just completed her grade 5 piano exam the month before. She hasn’t played since. She use to be a confident happy go lucky child. She was daddy’s girl you didn’t see one without the other. She’s lost her confidence. She was diagnosed with depression & complex ptsd & this breaks my heart. She has had to have therapy since 2018 first couple of years was art therapy on a weekly basis because she was just so young. As she’s got older now it’s been cbt therapy & still 5.5 years later she still having bereavement therapy as she is still absolutely devastated.
I don’t think we both will ever be the same. But it’s like every grief am sure, one day at a time Sweet Jesus. We are living with this & managing as best we can what else can we do. We are just devastated. My daughter doesn’t know the circumstances, daddy struggled with his asthma & COPD & sadly passed in hospital that’s been it. Why would I traumatise my daughter even more by telling her the horrific death he did encounter. The following day I went to identify Terry. I said I was going to see daddy. She said can you bring daddy his favourite packet of sweets. I said I would. When I got home she said did you see daddy what did he said. I said he loves you too darling.

Whatever country your reading our petition from Please Help Us. I’m doing what is morally right. I’ve been threatened to remain quiet. My legal team was threatened to the law society after our inquest, they reached out to Epsom general hospital with emails addressed to the two paramedics & the a&e staff to come forward regarding this new evidence re: this altercation with the a&e nurse and the female prison officer. No one ever did. I’m not even sure they actually received these emails that were sent. 

They those involved want me to sit back and say nothing. They want me to go away. I can’t do this I was brought up with integrity, morals & respect.  
We The family of Mr Roberts believe the overwhelming evidence against the prison officers many, prison nurses x2 that stood down the first code blue to accommodate their lunch break, placing my loved one in a single cell after just one nebuliser and sats levels that were dangerously low, locked in a single cell alone; police dr, agency prison dr & agency prison nurses 7, police nurse, Epsom General hospital A&E Dr x3 as well as Epsom A&E Nurses though this nurse 5’/5’2 in her fifties wasn’t interviewed or identified & Two paramedics. The two prison officers in the back of the ambulance one with a prison mobile phone in his pocket and didn’t use watching on as my loved one was dying. The other prison officer female who refused to remove the cuffs and chains when my loved one went into full cardiac arrest, As well as the other two incompetent & ignorant racist prison officers on the gate all belied by the coroners ruling, and that Terry did not receive justice. Why was both complaints from the ambulance paramedics re: the unnecessary waiting in Highdown prison carpark on a blue light ambulance an Emergency, an ambulance that was unable to leave & was being driven around a prison carpark slowly, whilst a male prison officer on gate duty was walking alongside the ambulance; his ignorant reply to both paramedics when they jumped the back of the ambulance at two different times shouting he’s deteriorating we need to leave now, male prison officer said I’ve already radioed through once & I am not doing it again, but yet High down prison confirmed they didn’t receive this response & said in court if we had known he was deteriorating we would’ve radioed for the two prison officers to open the gates immediately. 

The paramedics second complaint about being ignored by Epsom a&e depart staff when they arrived after driving at High speed from Highdown prison Surrey, the a&e staff were not on stand by when they should have been as this was an emergency & this emergency was radioed through to them well in advance. These two paramedic complaints were only produced during the inquest also. Again no one knew about this evidence either. Why was our inquest not stopped immediately! 
Our 4 week inquest was a blame game throughout, continuous lies & cover up! ‘I don’t recall answers. It wasn’t me it was her. It wasn’t her it was him.
‘The prison Dr asked you to follow up on Mr Roberts medication as he only had one blue inhaler which we knew the morning of his death was no longer working’, his the male agency nurse reply, ‘the prison dr didn’t tell me which day’.
Each & everyone of them had a Duty of Care! 
This most certainly is a Human Rights Act Article 2 The Right to Life which Terry’ was not given & Also A Human Rights Article 3 The Inhumane & Degrading Treatment he horrifically received. 
We are calling for a Full Public Inquiry into his death. Bring all the liars all witnesses & the outstanding witnesses that were never even questioned or interviewed into a criminal court setting with recorded cameras & whilst each witness is being cross examined let’s see if they can recall then. Let’s see if their stories change then or if anything matches up. 
This most certainly is a serious crime that has been committed by all involved in the inhuman treatment, unnecessary & continuous delays. It was never investigated properly from start to finish!
If I Remain Quiet in which These Individuals want me too, then they the 5 other interested parties & many within one interested party;  these individuals Have Got Away With Killing My Loved One. As Long As Our Heavenly Father Wakens Me Each Day I Will Continue To Speak as I Am Doing What Is Morally Right!
