A series of what appears to be distorted reports hound Senior Medical Oncologist Dr John Grygiel who has taken leave during his 'investigation' by NSW Health. As a fellow oncologist and who has worked with John over 20 years ago, looking at the data objectively, I believe that this attack has been politically motivated for the following reasons: 1. The Director of the Cancer Centre is NOT an oncologist but a Head and Neck Surgeon - so, he is not qualified to comment on what he perceives as 'errors' (neither is the Press) 2. Most Head and Neck patients receive chemotherapy with irradiation concurrently and not sequentially. This is part of a Multidisciplinary approach which may include radical neck dissection etc - or just palliative care support. We do not know the breakdown. 3. The majority of cases are treated with palliative intent - those treated with curative intent are usually treated aggressively but this is a small number. 4. For item 2 the US standard of care with carboplatin is usually with about AUC 6 if this is a single D1 dose (whilst Ozzies use AUC 4 and even AUC 1.5 (as part 2 of TPF) - according to EviQ, the NSW standard) . Calculated for a person with a body surface area of 1.8 m2, the dose quoted as 'incorrect' is not unreasonable. 5. For palliative intent therapy, the goal is NOT to cure but to palliative without causing much adverse effects, so, a standard lower dose is acceptable.
