My Grandad was took to the doctors with abdomen and chest pain and was prescribed antibiotics. A few days later, still in pain, the district nurse came out to see him and we were told to call an ambulance for suspected pneumonia. At the hospital my grandad was prescribed antibiotics for a water infection, although no tests were done for them to know he had a water infection. Few days passed and my grandad still wasn't appearing to get any better. So the family called an ambulance, in which we waited 4 hours for it to come. My Grandad was left in a cubicle in A&E from 4pm til around 2:30am the following morning, with no observations done or nothing! The following day we went to visit to find my Grandad strapped up with wires, uncomfortable and blood coming from his mouth, unable to breath. Although we've seen grandad in worse states, we knew something wasn't right, yet Grandad fought the worse 2 years ago, so this didn't seem too much to worry about. After all, he was in hospital, grandad was in the best possible place...right? While here, I looked through my grandads files and noticed the name on one of his records was in fact Albert Taylor not Albert Talbot! Around 5pm that day, 5th April 2019, we left the ward as it was dinner time for the patients, however my uncle stayed, as grandad didn't like to be left alone. We kept in contact with my uncle to see how grandad condition was getting on. About 8pm my uncle phoned, grandad looked a bit better. Around 10pm we had a call to say that my uncle was stopping, from then we knew something was wrong, otherwise the staff would have told him to leave. Around half an hour later, we had the call to get up the hospital as soon as possible as grandad was deteriorating rather quickly. When the family got there, grandad was laying in the bed, unable to move any part of his body. Doctors and nurses were coming in to check how things we're going, rather regularly. In the mean time, they explained how they would give my grandad some needles to make him more comfortable. After two needles, grandad was practically still the same....lifeless. We were told they were morphine and diazepam. Hours went by and no sign of any movement. As family went out for a breather, my uncle walked in to see a nurse giving my grandad 'the last injection' despite being told that the family were to be in the room when he had anything. Within 10 minutes, my grandad took his last breath, at 1:10pm 6th April 2019. 

It wasnt til later on, after all the shock, that the clock in the room was actually wrong, my grandad died at 2:10pm. A few weeks later my nan and mom went to register my grandad death, the registrar called out Albert Taylor!! 

We believe my grandad was treated as Albert Taylor and therefore that's why he died...through neglect! My Grandad was given Albert Taylor's medication and within a few hour's he was gone! 

Furthermore, when the family went to Walsall Manor Hospital to collect my grandad death certificate, as they were speaking about my grandad death, they were told that the same mistake had happened to another lady on the ward! 

Walsall Manor Hospital have made many mistakes, we believed our grandad was in the right place, that he would get better, but instead he died in the hands of the NHS, due to their failings.
