I'm Dr.Muthukumaran.MD Psychiatry working in periyakulam head quarters hospital, Theni district as assistant surgeon. I was appointed as per reference no.40914/E7/A1/2020-18 dated 15.07.2021 as per proceedings of the Director of Public Health and Preventive Medicine,  Dr.T.S.Selvavinayagam ,MD,DPH,DNB. I was appointed as assistant surgeon under category 2 of class 1 of Tamilnadu medical service in the sanctioned post in the pay matrix level 22 in time scale of basic pay Rs.56100. 

    It is for your kind information that Government of Tamil Nadu has done statutory deduction of Rs.295 for New Health Insurance Scheme in my monthly pay.(employee no: 19021151819, CPS no: 7000436762/MEDL) 

  I had underwent surgery for shoulder dislocation last week in Ortho one hospital, Coimbatore which cost around 2 lakhs .Prior to surgery I have applied New health insurance scheme,2021 (GO.Ms.No.160, Finance department Dated 29-06-2021) for claiming cashless process of my surgery. But Insurance company denied/rejected my insurance claim since I'm not regularised in service. But Rs.295 was deducted from my salary every month for New health insurance scheme. So I kindly request you to do the needful for claiming reimbursement of my surgical expenses(Rs.2 lakhs)
