Introduce a means-tested fee for all NHS services - keep the NHS afloat

The NHS is a valuable system. However, it is in crisis.
What if we could maintain the 'free at the point of use' service, improve the care, and increase the wages of NHS staff (they are, after all, saving lives)?
How can this be done? By implementing a small and means-tested fee for all NHS interactions. An interaction may be a visit to your local GP, an emergency trip to hospital, or a major surgical procedure.
Something like this: If you earn below 20k p/a you don't pay a penny. If you earn 20k-30k p/a you pay £20 for each NHS interaction. If you earn 30k-40k, you pay £30 for each NHS interaction. If you earn 40k-50k, you pay £40 per interaction. And so on...
Personally, as someone earning very little, I would be very happy paying a small fee for each interaction that I have with the health service, especially if it makes the NHS more sustainable.
I am NOT writing about an American-styled 'pay before we treat you' system. Any fees would be paid after the NHS interaction. Yes, this may allow a few people to abuse the system and not pay. However, I believe that most people (who can afford to) would be happy to pay a small, per-visit fee, if it means we can keep out precious health service up and running.
Please support this. The NHS will not survive in its current state. It needs increased resources, but these resources do not come out of thin air. This idea would ensure that those worst off in society continue to receive free care, and, that those better off in society contribute a modest sum towards ensuring a sustainable future for our health service.
