OSBN has started to interpret Standing Orders and Nurse Driven Protocols as "nurses diagnosing patients" and believes it is out of our Scope of Practice. Simple things like Bowel Protocol (patient hasn't pooped in 5 days, in danger of a bowel obstruction), Arrhythmia Protocol (new onset of A-FIB that needs to be addressed rapidly), PONV (patient is nauseous from medications or pain, actively monitored without prolonged QTC). We need to be able to perform simple interventions quickly and we should be trusted with the ability of responsible reporting to our doctors.

Changing the interpretation will likely remove bedside RN judgement, cause increased difficulty obtaining orders and increased intervention times, which could cause harm to the patient due to delayed treatment.

We know that we all received training on how to appropriately assess patients, are able to follow the outline of the protocol and update the physician accordingly. We are also realistic about how much time it takes to contact a physician with a full case load that may or may not respond to your page. We know they receive 100s of pages daily, causing alarm fatigue and missed communication opportunities. Let's not make our jobs harder or physician's days longer!

This change is NOT based on any harm or poor outcomes to patients, but is strictly being interpreted by the current law reviewers. OSBN needs to know this will negatively influence your practice as an RN in Oregon and potentially cause patient harm or delay in care!!
