On Saturday, March 11, 2017 at 1 am, our beloved family member, Crystal Clemons, passed away while under the care of Woodhull Hospital, in Brooklyn, New York. The cause of death is yet to be known, but the family believes she passed away from complications due to pneumonia, an infection she contracted while in the care of Woodhull Hospital doctors.

Crystal Clemons was 52 years old when she was initially admitted into Woodhull Hospital on Sunday, February 26, 2017. On that Sunday morning Crystal went into cardiac arrest while at home. EMT showed up soon after and upon regaining a faint heartbeat, transported Crystal to the Emergency room of Woodhull Hospital. She was later transferred to the Intensive Care Unit and for two weeks was not properly cared for. She received dialysis which caused her other organs to weaken, was not initially given a CAT scan to determine brain activity, and blood and saliva found behind her neck and cleaned by family members.  In addition, family was illegally contacted via telephone and while at work to be told the hospital staff would be unplugging machines leaving her to pass on her own without family consent.  Due to the lack of necessary care, which was not always provided, family members had to make demands and requests from doctors on call. Quite often, family was misinformed or not informed. For example, family was not informed that Crystal had pneumonia until two days prior to her death when it was already at Stage 3. The family later learned that she had it for several days and was even treated for it, but the antibiotics given did not assist in her health. Crystal passed away at 1:00 am on Saturday, March 11, 2017 after family members had to urge doctors on call to assist her. They remained seated as family members begged for them to check on Crystal who was gagging due to the increased infection, pneumonia, in both her lungs.

The family is seeking justice for Crystal's negligence. She entered the hospital for one reason and died from another. We want to make sure that Woodhull Hospital is investigated to ensure that no other families will have to endure the tragic experiences Crystal and her loved ones have experienced. A hospital is a place you go to in order to get better and Woodhull staff stripped Crystal of that opportunity.

We ask that you sign this petition and demand New York State Department of Health: Office of Professional Medical Conduct Intake Unit and the New York State Education Department: Office of Professional Discipline conduct an investigation into Woodhull Hospital of Brooklyn, New York. We want anyone who is sick and requires care to receive the love and attention that is necessary. We want Crystal to rest in peace knowing that those who meet these doctors and other staff do not have to meet their death as she did.

Crystal Clemons was a fighter and would have never given up a chance at life so easily. We, the family, know that had she been given the proper care, she would have had another opportunity at life, but she didn’t!  Please sign and help in her fight!
