Nurses & Nurse Case Managers in the State of Illinois whose role is to provide medical case management and advocacy for youth within the foster care system are reliant upon various parties to obtain and distribute information crucial to the child's wellbeing. Submission of medical information, nursing diagnoses, risk analysis reports are some of the essential documentation required for effective advocacy. If this documentation is not provided to the proper entities in a timely manner, it can be detrimental to the child. To make a change, this would require a large volume of people to collaborate and identify areas of improvement so that we can promote safe outcomes for these fragile youth in care.  We must work toward amending Juvenile Statutes within the legal system to advocate for these children within the courtroom to ensure safety in the child's plan of care by utilizing evidence-based practice.
