Bill for Transition Plans for Youth Leaving Child Welfare Care

There is a big problem at the moment with youth transitioning from the child welfare system to independence. The youth are unprepared and lack proper resources once they leave the child welfare system, which often leads to homelessness.
This problem could be fixed if they had caregivers or specific people to help the youth create transitional plans relating to them, their goals, and their needs. The youth would meet with these plan-makers frequently to monitor their progress in their plan, or adapt their plan if needed.
This plan would change the community's morale by prioritizing helping one another rather than succeeding individually. As a fellow youth myself, it is impossible to imagine the traumatizing experience this transition out of care entails.
If the youth were given the opportunity to have a transition plan and successfully followed it through, then the problem of youth homelessness could be greatly reduced. Help me turn this idea into a bill!
