My name is Stephanie Anderson and I am one of the upcoming chief residents of the Internal Medicine program at Grand Strand Medical Center in Myrtle Beach, SC. I am representing my program in regards to HCA's recent policy change regarding PTO days. During the time that all current residents signed their contracts for the upcoming GME year, we were allotted 20 days of PTO plus up to an additional 5 days to attend conferences. The new policy states we will be allotted 15 days of PTO plus up to 5 additional days to attend conferences. The change we wish to make, is to allow these 20 total days off (the 15 PTO + 5 conference days) to be all inclusive of sick days, vacation, and conferences instead of limiting it to only 15. We would like this change to reflect to all HCA GME programs nationwide. As internal medicine residents, we work very long hours. On inpatient services, we only get one day off per week and work anywhere from 60-80 hours per week. At my specific institution, we work extra hard during the holiday block coming in from 6:30am-7:30pm daily for 7 days straight to allow us to have extra time off during the holidays, which averages over the month to fall within ACGME duty hour requirements. Burnout is very high among residents. According to a study published in JAMA in 2015, 28% of residents experience a major depressive episode during training versus 7-8% of similarly aged individuals in the U.S. general population. We use critical thinking all day every day to provide our patients with the best care. But we can provide this care only if we care for ourselves first. These 5 extra days of PTO will make a significant impact in regards to helping to alleviate burnout among residents and we want them back.
