Give mothers the right to have the Consent to Relinquish form they signed

I am a mother who was forced to surrender my baby when I was 19 years old. At the time I wasn't told my rights, didn't see the paperwork, never gave my consent and have never seen the Consent to Relinquish form unless I was tricked into signing it. What happened was illegal as I didn't know my rights but I found out 23 years too late. Sadly my story isn't unusual and there have have been many mothers who were forced to surrender their babies from the 1940's to the late 1980's.
Under the Freedom of Information Act 1998 and the Data Protection 2005 I and other mothers have a right to see the Consent to Relinquish form. However I have been passed around different departments trying to grt access to see the form. Nobody wants to be made responsible for letting me see the form. There other mothers who are in the same position as me and all of us should be shown respect in being given the forms.
