People with severe mental health conditions die, on average, 25 years before the average population. Health Homes are one strategy to reverse this devastating trend. As authorized by Section 2703 of the Affordable Care Act, Pennsylvania can amend its Medicaid State Plan to create Health Homes- one-stop-shops where people with behavioral health conditions and other chronic conditions can get integrated care. The state plan amendment also draws down an enhanced Medicaid FMAP at 90%. Other states have moved on the development of health homes, and we need Pennsylvania- as one of the nation's leaders in behavioral health services- to do the same. This will save lives and save money for the Commonwealth.
