Prometric Test Centers operate a monopoly on academic testing for physicians and surgeons across the United States.  The majority of doctors are female (61% and 52%) in the age range of under 35 years and 35-44 years of age.  Breastfeeding initiation rates are around 95% for physician mothers, and all day test taking requires breast pump accommodations. 

Prometric Centers has a policy for Nursing Mothers (https://www.prometric.com/en-us/for-test-takers/prepare-for-test-day/documents/Test_Accommodations_for_Nursing_Mothers.pdf HOWEVER - they do not require standard private location options for all test centers.

Prometric Test Centers corporate policy should change such that every single local testing center has a private space to accommodate nursing mothers other than a restroom (as outlined by Dept of Justice FSLA workplace law for breast milk expression).  If Prometric Test Centers can force every local testing center to comply with other parts of regulating secure test location, then they can require a small private space for nursing mothers.
