In California, there are currently 2.7 million uninsured individuals and millions more who experience barriers to accessing quality care, such as the inability to afford medically necessary medications or gaps in healthcare coverage. The current multi-payer healthcare system is intricate and extremely flawed, as it is unable to offer equitable care to all individuals regardless of their income levels or citizenship status. The COVID-19 pandemic has only exacerbated this problem, as the number of individuals who have lost their employment and thus, their healthcare coverage has vastly increased. 

Assembly Bill (AB) 1400 will bring California extremely closer to achieving a single-payer healthcare system that ensures all California residents, regardless of income levels or citizenship status, have access to equitable, affordable, and quality healthcare. CalCare, or the California Guaranteed Health Care for All Act, guarantees access to quality and long-term care because it functions on the belief that healthcare is a right, not a commodity.


AB 1400 (CalCare) would establish a healthcare system that eliminates waste and matches reimbursements to the actual cost of care. CalCare is arranged to be an independent public institution governed by a nine-member executive board who are experts in healthcare delivery and policy. CalCare’s board will be representative of California’s diverse population and will be free of any conflicts of interest.


The implementation of CalCare would expand coverage to approximately 3 million more individuals residing in California and would ensure they have access to comprehensive health care coverage and have the freedom to choose providers. Beneficiaries would have access to all primary and preventative care services, including hospital and outpatients services, prescription drugs, dental, vision, reproductive health services, maternity and newborn care, and mental health and substance abuse treatment. Californians would receive services and benefits without paying premiums, deductibles, copays, or other out-of-pocket expenses. CalCare would remove barriers to care by creating a special project budget to fund facilities in rural or underserved communities. Lastly, long-term services and supports would be fully covered for medically determinable conditions, whether physical, mental, or due to age.
