Multi-drug Resistance Organisms (MDROs) are a global burden that takes the lives of millions and threatens even more. One mechanism by which these MDROs gain resistance is the development of enzymes that break down our only weapon against them Antibiotics.

The main enzymes we will be talking about today are Metallo-β-lactamase (MBL). MBLs are some of the hardest to fight as they break down one of the strongest antibiotics classes in the market which is Carbapenems resulting in MDROs known as Carbapenem-Resistant Organisms (CRO).

MBL-producing CRO can be identified through genetic testing and mechanism of resistance prediction through culture and sensitivity. Some of the characteristic Genes are NDM (New Delhi MBL), VIM ( Verona integron-mediated MBL), and IMP (active-on-imipenem). 

This MBL-Producing CRO can be eradicated with colistin-based regimens known for a high failure rate resulting in high mortalities. The second regimen is AZTREONAM-Ceftazidime/Avibactam which is known for high success in clearing these CROs.

The major problem is Aztreonam isn't available in the Egyptian market. A life-saving drug that could change the way we fight CROs isn't available in the market. 

This is a call to all pharmaceutical companies and Regulatory affairs. We are in desperate need of the release of Aztreonam in the Egyptian Market to help us withstand the Siege of MDROs and a call for all healthcare workers to stand together and work side by side to increase the level of patient care.
