Health Homes under attack in NYS budget!

Governor Andrew Cuomo has targeted the NYS medicaid funded Health Homes program for "Medicaid course correction." This is despite data that shows the Health Home program has improved the quality of care significantly for many with chronic health challenges, including a 20% reduction in potentially preventable emergency room visits, an 11.1% reduction in readmission to a hospital within 30 days of discharge and a 35% decrease in members with transmittable HIV.
The Health Home program was created by the Affordable Care Act as a State Medicaid benefit to coordinate care for people who have chronic complex health care conditions and are facing challenges in staying healthy.
New York State’s Medicaid health homes program is a key component of the health care delivery system, with care coordinators in the program helping individuals with multiple health challenges to better coordinate their care and improve health care access. New York State’s Medicaid Health Home program has consistently and successfully helped vulnerable and marginalized New Yorkers to better access outpatient medical care, concurrently reducing expensive emergency room visits and avoidable hospital stays through engagement, service coordination, housing placement assistance, and linkage to community-based social support services.
The program has sustained funding reductions already, with cuts of $20 million in the adopted budget for each of the last several years. iHealth and its partners have submitted recommendations to further improve the program - those recommendations should be adopted before making any further cuts. We urge the legislature and Governor to reject further cuts to this program in the final adopted budget.
