This is a formal petition for Frank B. Smith III, a 100% service connected Vietnam Veteran Honorably discharged from the United States Air Force regarding his need for adaptive work to be done in his home. He has lived in his home in Homewood, IL for seven years now and throughout his time living here, he has not been able to safely leave and enter his home due to his disability. As the picture shown above presents that when he has to leave for doctor appointments, he fears the risk of becoming injured. This petition will implement the necessary steps to be put in place for him to live normally in his home. Currently, as it is, there is no way for him to safely egress or ingress his home. There is a need for wheelchair modifications to access every room in his home. The individual names that have signed this petition can attest to the living condition mentioned above or can stand in agreement that he is need of assistance in making his home more conducive so that he can egress or ingress his home as necessary.
