    TXA sounds like a great drug and I hope it becomes more readily available. There are some problems that will have to be confronted to make it more feasible like deciding when to use it related to its risk for thrombotic events such as strokes, pulmonary emboli, and heart attacks and so on. In order to make such a decision you need not only the right diagnostic plan but one that works well. I've worked a long time in healthcare and in reality i've seldom seen systems and protocols work well with this type of situation. When you have someone who comes in from an auto wreck there often has to be radiology intervention including cat scans and magnetic resonance imaging. I have seen patients wait for long periods of time and often much longer than the six hours suggested for head injuries by the aha and others. These delays are often caused by cost cutting corporate style mentality that is especially prevalent at night and is mostly responsible for understaffing and/or operating with  on call personnel that leads to logistic problems. In order to change such scenarios it is necessary to show corporations how they can profit which is their main motivation. By being proactive versus reactive they could save more lives and charge accordingly but that can be a tough mission depending on management style and political environment. I do think txa would be a great addition to present life saving technology and it needs to be more available. Steve Jaubert

    Panhandling is going to be whether there is homelessness or not. There are people holding out the cup in good times and bad times. I don't think it makes homelessness worse.
