Everyone wants control over their own life, but to accomplish most things in life, people have to work together. How they decide what and how is the definition of politics. I consider each such group a separate government. How that particular government is organized, who decides and how the others hold that decider responsible, is the fine grain of freedom or tyranny, and differs in each case.

Turning Japanese? We Could Do a Lot Worse

Japan is a single payer system, Frontline had a great in depth discussion of many systems. Of all, only the UK was a "government all the way" system. They go into great detail about the good and bad of the Japanese system.

I also did a Blog post about Sicko and Gapminder that bears updating as Gapminder has improved a great deal, but points out many facts about American care vs Civilized care.
